Digital forms are a great tool for collecting and storing client information, creating contracts, optimizing registration and saving time.

With Arbox you can create different types of digital forms - like waivers,rules and regulations, feedback and satisfaction, health declarations and more.
‍

The forms can be digitally signed through the client app during the client’s registration process, or through a unique link.

Once the form is complete, it will be automatically stored in the client’s profile for easy access. You can also manage forms through the “signed forms” reports, where you can see which clients have yet to fill out the forms and issue reminders.

Creating a Form Type

Before we go on to create our form, we need to determine its type.

1. Go to the “digital forms” page through the side menu.

2. Click on the settings icon.

3. Click on create.

4. Write the form type name, set an expiration date and choose if it will be meant for clients and leads or your staff

How to Create a Digital Form?

1. Stay in the “Forms” page.

2. Click on the “create digital form” button.

3. Now we will create our form template.
First, fill in the form’s details - name, type, whether you would like it to apply only to certain memberships and more.

4. Write the form’s content according to its type. You can also add a questionnaire to the form if you wish to do so.

5. Define the details the clients will need to provide in their signature, and if it will be signed digitally or by checking the appropriate box. When you’re cone, click on “save template”.

If you want, you can mark this form as mandatory, meaning the client will be obligated to sign it before registering to your business.

Now you can attach this form to an automated message. For example - a message containing a rules and regulation form will automatically be sent out everytime a new client is created in the platform.
